Discharge Orders:

1 Discharge from hospital to home ad lib

1 Diagnoses: resolving pneumonia, diabetes, HBP

2 Resume normal activities

3 Remove intravenous catheter, if present

4 remove urinary catheter, if present

5 oxygen by nasal canula at 2 L/min

6 regular diet

7 Meds:

a levaquin 250 mg qd #14 orally

b albuterol MDI 2p qid prn sob wheezing or tight chest

c prednisone 20 mg #35 3 qd x 3d, then 2 qd x 3d, then 1 qd x 3d, then ½ qd x 3d, then as directed

8 continue pre-hospital meds (patient has sufficient):

a accupril 10 mg qd

b glyburide 2½ mg 1-2 qd as directed

9 f/u appt w/ me in one week
